Register With The Practice?

All new patients must see a nurse to register with the practice. You will be offered a “New Patient Interview”, which involves some simple health checks and allows us to learn about any previous medical problems and any allergies that you have and any medication you are taking. You are free to choose to see the doctor you prefer but in general it is better to stick to one doctor, especially when dealing with a continuing problem.

How do I...
Change My Personal Details?

Please let us know as soon as possible if you change your name, address or telephone number. It is useful for us to have your mobile phone number if you have one. If you move outside the practice area (see back cover of this booklet), you should register with a doctor who is local to your new address.
