Obtain A Repeat Prescription?

Repeat prescriptions may be requested by:

(1) Using the stub from your previous repeat prescription to mark the repeat medication required and send to reception or

(2) Faxing your prescription request to the surgery or

(3) Emailing your repeat prescription request to: 3VMP.prescriptions@dudley.nhs.uk

You will need to include your FULL name and date of birth. Please list EACH item you require - medication/product name, type (tablets/syrup/cream, etc), strength, dose, code, etc.

NOTE: You should clearly indicate whether you wish to collect the prescription from either Stourbridge or Wollaston surgeries or if you prefer, it can be sent to Amblecote Pharmacy or Boots, Wollaston. If youdo not indicate a preference, the prescription will be available for collection from the Stourbridge surgery.

Please allow 48 hours for the doctor to check and sign your prescription. If you wish your prescription to be posted please provide a stamped addressed envelope. If you wish to use a chemist’s services, you must make these arrangements yourself with the chemist of your choice. We do not accept repeat prescription requests by telephone.
